Teeth whitening is becoming an increasingly popular and none invasive cosmetic treatment for patients who just want to improve on the whiteness of their teeth.

Unfortunately, all types of teeth whitening system can cause some degree of sensitivity to your teeth, some more so than others. This sensitivity is reversible, and is related to the concentration of the whitening system, the length of time applied to the tooth, and the dehydration levels produced by the treatment.

At Hornsby Dental, we offer the latest cosmetic dental teeth whitening system: SDI is proud to present their 100% Australian made whitening system PolaOffice 2, which utilises a powerful blue-light and high potency whitening gel.

We do not use laser as our whitening light because researches have found that blue-light can be just as effective in offering an equal whitening effect without excessive dehydration which can be caused by some high-powered laser whitening systems in the market. Excessive dehydration can cause increased sensitivity of your teeth.

This treatment can be repeated up to 2 to 3 times depending on how white you would like to make your teeth. There is a healthy limitation to how white your teeth will become, and we do not advocate for excessive whitening.

Also, there are certain types of discolouration which cannot be improved from standard whitening, such as leaky old fillings. Your consulting dentist and hygienist will be able to advise you on whether professional tooth whitening is suitable for you.

If teeth cleaning is required for plaque and calculus removal prior to whitening, your dentist/oral health therapist will begin by doing so, and then carry out 3-4 applications of teeth whitening.

We also offer a take home option that can be used alongside in-chair whitening for improved long-term results or used as a stand-alone option for gradual whitening. Your dentist/oral health therapist will provide custom fitted trays and whitening gel to be used for 30 min/day for 1-2 weeks. Top up gels can be provided after initial treatment for maintenance of whitening results.

Whitening carried out by dental professionals is a very safe and effective way of achieving rapid whitening results. The side-effects listed below are most often mild and temporary, but provided to best manage all expectations.

  • Tooth sensitivity:

Any teeth whitening treatments are likely to cause some degree of sensitivity due to the use of concentrated hydrogen peroxide formulations. This sensitivity is reversible and subsides in the days following treatment, however if you are someone with already sensitive teeth, it is not advised that you undergo whitening treatment as it can become quite uncomfortable. Using a sensitivity toothpaste following treatment can reduce this side-effect.

  • Unpredictability of results:

Unfortunately, we are unable to guarantee a certain degree of whitening as each person’s teeth will react differently, however our experienced clinicians will be able to determine your suitability for professional in-chair or at-home whitening. Artificial tooth surfaces (crowns, veneers, fillings) and roots of teeth cannot be whitened and may need to be replaced following whitening to achieve a similar shade match.

  • Tooth dehydration:

Immediately following whitening, teeth can display a white, patchy appearance as a result of dehydration (what mostly contributes to sensitivity). This is temporary and within a few hours begins to subside as the tooth rehydrates naturally.

  • Gum irritation and burns:

Usually mild and temporary, however precautions are taken to minimise this such as covering the gums during in chair whitening or using custom fitted trays for take-home options. Applying Vitamin E oil in the case of gum burns can minimise irritation.

For longer lasting results, it is best to avoid both acidic and dark coloured foods & drinks especially in the days following treatment. These can include beetroot, berries, tea, coffee, red wine, soft drinks and various fruit juices.

Smoking and other tobacco products can also reduce the effects of whitening, so it advised to limit their use after whitening. Maintaining regular twice daily brushing and daily flossing, as well check-ups and cleans every 6 months will further improve the longevity of your bright and white smile.

Dental emergencies can arise at any time, sometimes stemming from pre-existing dental conditions or often because of accidents or injuries. Such situations can be scary and stressful however having a good understanding of possible causes of pain and home care management until professional treatment is sought can go a long way in improving prognosis of most dental conditions.

Tooth or gum pain can quite easily get in the way of things we love doing most – enjoying a slice of cake with family, engaging in conversations with friends and getting a good night’s rest. Whether sharp and shooting or dull and aching, tooth pain should not be ignored, rather resolved as soon as possible.

At Hornsby Dental, we know that toothaches can develop suddenly and out of nowhere, and we know how debilitating this pain can be sometimes. Injury to the teeth or gums can also be a great source of pain and distressing. Not only do these cause significant pain and distress, dental pain and injury can potentially have a serious effect on your dental and overall health.

Tooth pain is usually an indication that your tooth has become infected, which if ignored, can progress to become an abscess or a life threatening emergency. Any tooth or gum pain or injury should be addressed by a dental professional as soon as possible.

Some common causes of dental pain include:

A condition whereby acid producing bacteria in plaque attack the dental hard tissues. If left untreated, this decay can progress to underlying layers beneath enamel, eventually reaching the nerves and blood supply of the teeth.

This is usually the stage at which pain is experienced, thus emphasising the importance of regular dental check-ups for early detection and prevention.

In the case where bacterial biofilm remains in the mouth for extended periods of time, an inflammatory response can be triggered by the body, causing destruction of the teeth’s supporting structures – those being connective tissue and bone. In advanced stages of gum disease, one may experience bleeding gums, pain and discomfort while biting and chewing, and even mobility of teeth.

  • Dental trauma:

If injury to the mouth is encountered, pain may be experienced especially if the teeth have become either knocked out of position or broken. Whether or not there are visible signs of injury to the teeth, it is a good idea to still visit the dentist near you to rule out any signs of trauma that may not be visible without radiographic evidence.

  • Tooth wear:

Grinding, acid erosion and abrasive brushing technique can wear away enamel and expose the underlying layer of dentine, resulting in sensitivity to cold foods and drinks. If this is consistently an issue for you and can be identified as such, then it is not considered a dental emergency but conveying this to your dentist/ hygienist at your regular check-up appointment can help in identifying ways of managing symptoms.

Although not all the above are considered dental emergencies, your severity of pain (in the absence of other symptoms such as abscesses or facial swelling) will often be a good indication of whether emergency care is required. Your dental clinician will thoroughly assess your symptoms and examine the problem site to advise you on your best treatment options.

Management of an avulsion (knocked-out tooth) will differ depending on whether it is a primary (baby) or permanent (adult) tooth that is affected. Avulsion of a permanent tooth that results from an accident or injury is considered a dental emergency and the successful outcome of the tooth depends on how quickly a tooth is reimplanted into its socket.

According to the International Association of Dental Traumatology guidelines, immediate first aid for a knocked-out tooth includes:

  • Washing the tooth under cold running water for no more than ten seconds while being careful to avoid touching the root.
  • Correctly positioning and replanting the tooth into the socket and biting onto a cloth to hold it in place.
  • If unable to replant the avulsed tooth, it should be soaked in milk or the patient’s own saliva in a transportable medium and taken to the dental clinic.
  • Alternatively, if conscious, the tooth can also be left in the patient’s mouth until assessment by a dentist but never left to soak in water.

Treatment of avulsed or mobile teeth following dental trauma is case specific, however acting in a fast and calm manner until your dental appointment can give the tooth the best possible chance at survival.

Treatment of a chipped or broken tooth will depend on severity of the fracture and how much of the tooth is involved. If possible, the lost fragment should be brought with you to your dental appointment as in some cases, it can be seamlessly bonded onto the fractured tooth.

If injury involves laceration of the oral soft tissue, first aid care involves applying pressure with gauze until haemostasis is achieved or upon arrival to the dental clinic. In the case of a small laceration, your dentist may advise you to keep applying pressure to promote healing, however larger lacerations may require sutures.

If tooth decay and gum disease are not addressed early, the advanced stages of either can result in localised infection around the affected teeth. Emergency dental care treatment should be sought for this because if left untreated, can result in fever, pain, lethargy, mobility and/or tenderness.

In more severe cases, spread of infection to surrounding tissues can result in a diffuse inflammatory response characterised by swelling, redness, and warmth of overlying skin. Other symptoms include pain, fever, malaise, tenderness to palpation, lockjaw, and difficulty swallowing.

If the infection has systemic involvement, airways are compromised, a significant inability to swallow, or severe dehydration is detected, hospital admission may be required.

Root Canal Therapy

Injuries or infections inside a tooth can kill its nerve, creating excruciating pain during the process. Not always are we ready to take this bad tooth out. If appropriate, we can perform root canal therapy which provides your tooth an opportunity to stay in service for longer. Root canal therapy sterilises the inside of the tooth to rid of the bad bacteria and infections. Once a root canal therapy is completed we strongly recommend a crown to protect what is left of the tooth. Unfortunately not every tooth with a bad infection can be saved from root canal therapy, but we will do all that we can to save your tooth.

Within each tooth, dental pulp is present. It is composed of connective tissue, blood vessels and nerves, providing the tooth with a supply of nutrients and protection. In some cases, pulp tissue may be infected by bacteria or decayed debris, causing an abscessed tooth.

Abscess, a pocket of pus, can spread past the ends of the tooth’s root and cause swelling to the face, neck or head, as well as bone loss at the tip of the root. Causing mild to severe pain, root canal treatment is the best way to save the tooth and prevent further spread of infection.

Root canal therapy is a procedure used to repair a damaged tooth by removing the infected or inflamed pulp and replacing it with a filling. Known as an endodontic treatment, the procedure involves drilling into the affected tooth and removing dental pulp which resides within the soft centre.

Once all the bacteria, debris and pulp are removed, the canal space is permanently filled and is sealed helping to prevent further reinfection of the tooth.

Common symptoms that show that a tooth needs root canal therapy include:

  • Severe pain during biting or chewing
  • Lingering sensitivity to hot or cold
  • Tooth discoloration
  • Chipped or cracked teeth
  • Swollen or tender gums
  • Swelling of face or neck

Sometimes there are no symptoms or little experiences of pain. This is usually because the pulp has already necrosed, resulting to complications such as:

  • Localised bone loss, or
  • Loss of tooth

An abscess may still be present despite the tooth not hurting.

In cases of tooth injury caused by accidents or sports injury, neighbouring teeth may suffer additional, unnoticed injury which can only be detected by a dental exam. It is important to see an emergency dentist Sydney after experiencing injury, even if there are no visible chips or cracks.

As the anaesthetic wears off, there will usually be tenderness surrounding the area and mild soreness in the jaw for the next following days. During this time, it is important to take medications as prescribed as there will be temporary experiences of sensitivity and discomfort.

The tooth can be saved with a carefully performed root canal therapy and avoid the loss of the tooth.

 

MEASUREMENT

Measurement: The root canal therapy is essentially the treatment to rid infection inside the pulpal/nerve space within the tooth, and to seal the tooth from further reinfection into the root tip of the root. This needs to be performed very carefully and should always have a “rubberdam” isolation field to avoid the re-introduction of bacteria from within the mouth into the canal spaces. Without rubberdam, the treatment is considered compromised. In this image the dentist has used a clamp to lock the rubberdam to the tooth and is measuring the sealing length for the final root filling.

 

POST-OP

Post-op: A well-obturated/filled root canal therapy. The tooth is symptom-free and will have a crown placed on it to seal the tooth from any potential fracture and decays in it. Studies have shown a well-placed dental crown can provide superior seal and, if possible, should always be placed on the tooth after the root canal therapy is completed with the tooth being symptom-free.

Removal of Stains

Staining and discolouration of teeth can impact one’s confidence levels and be a major source of insecurity for many. Stained or discoloured teeth can cause social embarrassment or psychological trauma.

Perhaps you’ve experimented with different toothpastes or bought home bleaching kits to whiten your smile, only to find that these options fall short of your expectations. Fortunately, your dentist can provide suitable cosmetic treatments with good long-term results.

Different types of stains are removed in different ways, but whatever the cause of staining, your dentist will be able to discuss all your treatment options during your dental consultation so that you can have the smile of your dreams.

There are several types of stains:

Surface stains occur between teeth and on the surfaces of crooked teeth. They usually look brown, and often they are caused by coffee, tea and tobacco.

To prevent surface stains, cut back on coffee, coca cola, tea and red wine, and avoid smoking. Visit your dentist regularly for professional cleanings, and brush and floss regularly.

Soft deposits are caused by plaque or tartar build-up. Often these start from bacteria, and are usually due to inadequate oral hygiene. They can look like dark and whitish areas around the gum line, and can be removed after dental scaling and polishing.

Tetracycline stains can be yellow, brown or gray. Multiple in-office bleaching can create a much whiter smile.

Intrinsic stains look like white splotches or bands of brownish gray on the tooth surface. They may be due to genetics, disease, medications or advanced decay by old silver fillings.

There can be many different causes of staining, not all being related to one’s diet. Stains can either be ‘extrinsic’ – on the outer surface of tooth enamel, or ‘intrinsic’ – in the inner layers of the tooth. Often extrinsic staining is more easily treated than intrinsic stains through regular cleans and polish or even through teeth whitening.

  • Extrinsic staining:

It is often foods and drinks that contribute to this type of staining. These include tea, coffee, red wine, dark coloured fruits such as berries and natural fruit juices.

These foods and drinks contain ingredients known as ‘tannins’, which cause colour compounds to stick to teeth and result in staining. Smoking and the use of tobacco related products as well as improper/infrequent oral hygiene habits can also result in extrinsic stains.

  • Intrinsic staining:

Common causes of this are excess fluoride exposure, trauma to the teeth, certain illnesses and certain medications including tetracycline antibiotics either during pregnancy or in early childhood development years. The tooth layer beneath enamel known as ‘dentine’ is naturally yellow and sometimes a thinner overlying enamel can mean more yellowish appearance of teeth.

  • Age-related staining:

This is a combination of both intrinsic and extrinsic staining. As we age, our dentine becomes more yellow.

This combined with the thinning of enamel from everyday tooth wear can cause the yellowish appearance to become more pronounced.

Treatment Options

Polishing is the simplest way to remove stains. It is a painless procedure where your dentist applies a paste to the surfaces of your teeth using a rotating instrument. Deeper intrinstic stains may require for advanced treatment such as microabrasion.

This is carried out during your regular dental check-ups and cleans. Your dentist/oral health therapist will assess the amount of staining on your teeth and remove extrinsic stains using either polishing paste or by using an air polishing system.

Polishing is best if you:

  • Have only slight surface staining
  • Want a natural look
  • Want to spend a small amount of time and money

Teeth Whitening involves using an oxidising agent to lighten the teeth, and is best for moderate surface and intrinsic stains. Your dentist may recommend in-office or home bleaching depending on the nature of your stains.

This option is available as either a 90 minute in-chair whitening session or as a take-home option involving custom fitted trays. While teeth whitening will lift certain stains and discolouration, it may not be the most effective treatment option particularly for intrinsic stains. Your dentist/oral health therapist will discuss this during your appointment.

Bleaching is best if you:

  • Have mild to moderate tetracycline, fluoride or trauma-related staining
  • Don’t want to spend too much money
  • Are happy with the shape of your teeth
  • Prefer a more conservative, noninvasive approach

Make the most of bleaching:

  • Tell your dentist if you have both crowns and natural teeth that you want to be lightened
  • Avoid consuming citrus fruits, juices, soft drinks and antacids after bleaching
  • Decrease your intake of refined sugars before and after bleaching

Bonding involves applying composite resin to the existing tooth, and can mask many types of stains to create a natural-looking and attractive smile. However, bonded teeth tend to stain easily and require periodic repair.

Bonding may be best if you:

  • Have white or brown spots or staining due to excessive wear or silver fillings
  • Are not a heavy smoker or coffee drinker
  • Are willing to take extra care of your new smile
  • Want a less expensive and less invasive option (compared with veneers or crowns)
  • Take care of your bonded teeth:
  • Do not chew ice or bite your fingernails
  • Brush regularly
  • Floss daily, but pull floss out horizontally, not vertically
  • Have your teeth cleaned at least four times yearly
  • If you clench or grind your teeth at night, use a nightguard
  • Minimise consumption of coffee, tea, colas and sugar
  • Don’t smoke
  • Cut your food into small pieces and use your back teeth to chew

Dental Veneers are layers of material that can be placed over the natural tooth to improve your smile aesthetic. Composite resin is a material choice for veneers however when compared to porcelain veneers, they can stain more easily and may require more repair.

Porcelain veneers are longer lasting and less prone to staining however they are also more costly. It is one of the most exciting techniques in cosmetic dentistry today.

It involves bonding a thin laminated porcelain veneer to the tooth surface. Its primary advantages are the beauty and durability of the material.

Porcelain doesn’t stain like composite resin, and remains attractive for a much longer period of time. In addition, porcelain veneers can improve not only the colour but also the size and shape of your teeth for a total smile makeover.

Porcelain veneers are best if you:

  • Want a significant change in the colour of your teeth
  • Are willing to undergo slightly more invasive and time-consuming procedures
  • Can afford the higher cost of treatment
  • Want highly aesthetic results that will last longer

Placing crowns is a more costly and time-consuming option compared with other solutions to staining. However, it can produce near-perfect aesthetic results.

Crowns require removal of natural tooth structure, so are usually only recommended for stained teeth that have additional aesthetic and/or functional problems.

Although this option is more costly and time-consuming, the results that can be achieved are superior to other treatment options. This treatment is suitable for those looking for a complete smile makeover but is not recommended for all cases since it requires extensive removal of natural tooth structure.

It is mainly used when aesthetic concerns are adjoined with functional problems.

Crowns may be best if you:

  • Have many large defective fillings

During your examination appointment, x-rays and photographs may be taken to ensure there are no underlying conditions that first need to be resolved prior to embarking on your orthodontic journey. Computer-generated visual aids will be able to give a good indication of what the final result will closely resemble.

Invisalign:

Invisalign is the preferred treatment option amongst young adults, utilising clear plastic aligners for the very subtle and comfortable movement of teeth. These clear aligners are removable and are changed every few weeks but must be worn every day and night except for when eating or brushing and flossing.

The length of treatment and number of aligners required will vary between individuals and their degree of misalignment.

In more complex cases, Invisalign may not be an ideal treatment option however a consultation with your dentist will provide you with more tailored information on the most suitable options for you.

At Hornsby Dental, Dr Philip Song offers fixed orthodontic treatment (braces). A choice of conventional brackets, or ceramic brackets can be selected.

A thorough consultation and examination, as well as photographic records are taken during an initial consultation. These records are used to analysis and plan orthodontic treatment.

At Hornsby Dental, traditional dental braces cost Sydney starts at $4500 but can be as high as $9500+ depending on treatment complexity and whether metal or ceramic brackets are chosen, including all adjustment appointments as well as retainers upon finishing treatment.

The Invisalign cost has interest free payment plan from $25/week and is for very minor alignment issues but like traditional braces, this will vary based on the complexity of each case.

The exact cost of treatment is determined by the complexity of the case rather than the time it takes to complete. Usually a 30% deposit applies at the commencement of orthodontic treatment and the balance is paid at regular intervals every 1-2 months.

You also have the flexibility of paying for the full cost upfront in one payment prior to the start of treatment, and we offer a discount of 10% to the total cost (e.g. 5,500 – 550 (10% discount) = 4,950). Ceramic brackets are available at your choice for a slight extra cost.

Happy Gas is one of the most traditional forms of inhalation sedation agents for the relief of anxiety, fear, or stress. At Hornsby Dental, we offer Happy Gas sedation for our anxious patients. It is safe for most children and adults, and you can usually recover from it rather quickly. Its rapid offset means that you will be safe to drive after a short rest after the treatment.

If you have never had Happy Gas for any types of medical/dental treatment, and you do wish to receive it for your dental treatments with us, we strongly suggest that you come in for an initial consultation where we will discuss as well as test the effect of happy gas with you.

The reason being, much like any other medications, not everyone reacts to happy gas the same way, because we are all different, anatomically and mentally. What this means is, your level of tolerance may be different, and your response to sedation may be different. Some enjoy it, some may not, and some may require a higher concentration, while others do not.

At the initial consultation for happy gas, we will test a suitable level for you, as well as run you through some basic information you need to know before proceeding with happy gas for your next dental visit. This way, you will experience what it feels like for your comfort.

Dental Bridges are a type of dental restoration that is designed to replace a missing tooth, when dental implants or dentures are not suitable. It utilises the adjacent teeth as abutments to provide support of a pontic (bridge) to fill in the missing tooth/teeth space.

It offers some distinctive advantages:

  1. It replaces a missing tooth/teeth without surgery as required in a dental implant, as well as a significantly shorter treatment time.
  2. Is a “fixed” restoration unlike that of a removable partial denture.
  3. It is less costly than a dental implant.
  4. It has long-term studies to support its use in dentistry.

Unfortunately, because it does require support from adjacent teeth, often dentists will have to cut into them to prepare space for ceramic thickness, but with modern high strength materials, less cutting is required comparing to older materials.

At Hornsby Dental, we believe that dentistry should not be over-priced, and that dentistry fee should be transparent. A printed treatment plan will be provided to you with the dental bridge cost of each of the steps involved. We use only local suppliers and believe in keeping strict quality control.

A dental bridge is a fixed option to restore a missing tooth. Similar to a removable partial denture (plates), it utilises the adjacent teeth as abutments. The difference is, the bridge is cemented in and does not need to be removed.

It involves reduction of the abutment teeth with a crown preparation to accommodate for the high strength framework material and veneering ceramic. The disadvantage of this option is the necessity to prepare the abutment teeth, however it is an alternative option to replace a missing tooth when surgical implant or removable dentures are not suitable.
